Summary

  • Approvals on the storage ports. With a storage backend configured (feat(storage): storage backend by URL; session stores on tinystoragedrivers; bump tinyagents + tinyflows #7168: OPENHUMAN_STORAGE_URL or [storage] url), every function in security::approval::store and store_flow_trust reads and writes the backend's document store instead of approval.db. That covers pending requests, decisions, expiry, the execution outcome, recent-decision audit, session purge and per-flow trust.
  • Scoped to the acting agent. Records live in the agent's storage scope (storage::scope_for_agent, the same mapping the session store uses). A single-user host with no acting agent uses the local scope. In SaaS mode a call without an agent is refused; it does not fall back to a shared scope.
  • At most once, across processes. Deciding, expiring and recording an outcome are compare-and-swap writes on the record's version. Two processes on one database can't both decide a request, expire it twice, or overwrite an outcome.
  • Desktop unchanged. With no backend installed, approval.db is used exactly as before; the SQLite code paths are untouched.

Problem

Solution

  • approval/store_documents.rs (Docs): a typed repository over DocumentStore. It uses two collections, approvals and approval_flow_trust. The decision, outcome, args and source context are kept as the same string fields the SQLite rows hold, plus an expires_ts epoch field so expiry can be a range query.
    • decide, expire_stale and record_execution go through one update() helper: read, check the state, write with Precondition::Version, and retry on conflict. A request that is already decided or missing returns None/false as the SQLite store does.
    • A flow-trust record's id is a length-prefixed (flow_id, tool_name) key, so ("a/b","c") and ("a","b/c") cannot collide.
  • Dispatch. Each public store function first calls store_documents::current()?. That returns Some(Docs) when a backend is installed, with the scope resolved from the current CoreContext, and the store then serves the call from documents. Otherwise it falls through to the existing SQLite code. Bus publishing for expiry was extracted into publish_expired() so both paths emit the same events.
  • storage additions:
    • current_scope() / scope_from(agent, saas) implement the fail-closed rule.
    • block_on runs a storage future from the store's synchronous API on one shared runtime thread (tinystoragedrivers::Blocking); block_in_place would panic on a current-thread runtime.
    • current_scoped() combines the two.
  • Trade-off: no import of existing approval.db rows into a newly configured backend. Pending approvals expire within ten minutes and the audit trail stays readable in the old file; a one-shot import can follow if a desktop ever switches backends.

Submission Checklist

  • Tests added or updated:
    • store_documents_tests.rs (12): round trip and ordering, a duplicate id refused, decide exactly once, eight concurrent deciders with one winner, expiry to deny once, outcome only after a decision and only once (error capped), recent decisions newest-first, session purge keeps decided rows, pre-authorization is audit-only, flow trust grant/list/revoke and key collisions, two scopes isolated, an unreadable source context reads as absent.
    • storage/mod_tests.rs: the scope follows the agent and fails closed in SaaS mode; block_on from sync code and from inside a runtime.
    • tests/storage_approvals_e2e.rs: the public approval::store API on an installed memory backend. It is its own test binary, so the process-wide storage slot cannot reroute the lib's SQLite approval tests.
  • Diff coverage ≥ 80%: CI Fast (EX63) passed the changed-line coverage gate.
  • Coverage matrix updated: N/A, no user-visible feature change (the desktop path is unchanged).
  • Affected feature IDs: N/A.
  • No new external network dependencies: tests use the memory driver.
  • Manual smoke checklist: N/A, default desktop behaviour unchanged.
  • Linked issue: N/A, part of the storage-drivers migration.

Impact

  • Desktop / CLI / TUI: no change without a storage URL.
  • Cloud / SaaS: approvals persist in the configured backend (MongoDB), isolated per agent, safe with several replicas on one database.
  • Dependencies: the tinystoragedrivers facade gains its blocking feature (a tokio runtime thread, no new crates).

Behavior Changes

  • Intended behavior change: with a storage backend configured, approvals persist there, scoped per agent.
  • User-visible effect: none on the desktop.

Parity Contract

  • Legacy behavior preserved: every public approval::store signature and return contract is unchanged; the SQLite path is untouched when no backend is installed.
  • Guard/fallback/dispatch parity checks: the document path mirrors the SQLite semantics (already-decided → None, outcome before a decision → false, expiry → Deny, the 512-char redacted error cap) and the tests assert them.

Summary by CodeRabbit

  • New Features
    • Approval data can use a configured storage backend, while the local database remains available when no backend is configured.
    • Approval records and trust settings are kept within the applicable storage scope, with safeguards against conflicting decisions across processes.
    • Approval storage follows the acting agent’s scope, and uses a local scope when no agent is active outside SaaS mode.
  • Bug Fixes
    • Expired pending approvals now publish expiration events consistently across storage options.

Add current_scope and scope_from to resolve the acting agent's storage
scope, refusing a shared scope in SaaS mode when no agent is present, and
current_scoped to bind the installed backend to it. Add block_on, a
shared-runtime bridge letting synchronous domain stores call the async
storage ports without block_in_place, which panics on current-thread
runtimes.

Add a store for document approval records so pending and resolved
approvals can be persisted and queried. This provides the backing
storage needed for the document approval workflow.

Auto-committed-on: dragonfly
Co-authored-by: Medulla <medulla@tinyhumans.ai>
When a storage backend is configured, approval operations now go through
the tinystoragedrivers document port under the current storage scope
instead of the local SQLite database, falling back to approval.db when no
backend is set. Expiry sweeps also publish stale-card events from the
store since no live gate is in scope during a sweep.

Auto-committed-on: dragonfly
Co-authored-by: Medulla <medulla@tinyhumans.ai>
Add tests for the approval store's document handling, exercising the
persistence paths that were previously untested.

Auto-committed-on: dragonfly

Extract the process-wide backend holder into a Slot type so tests can
install and clear a private backend without rerouting storage calls for
other tests running in the same process. The approval store tests were
also adjusted to drop a test that mutated the shared slot and to cover
the missing-error path instead.

Auto-committed-on: dragonfly
Co-authored-by: Medulla <medulla@tinyhumans.ai>
Add a Cargo test target for the new storage approvals end-to-end suite. It
installs a backend into the process-wide storage slot, so it must run in its
own process to avoid rerouting other suites' approvals.

Auto-committed-on: dragonfly
Co-authored-by: Medulla <medulla@tinyhumans.ai>
Re-export MemoryStorage from the storage module and extend the approvals
end-to-end test to assert the classic database is used once the configured
backend is cleared.

Auto-committed-on: dragonfly
Co-authored-by: Medulla <medulla@tinyhumans.ai>
The pending helper now builds its fixture through PendingApproval::new instead of
constructing the struct literal directly, dropping the chrono import that was
only needed for the created_at field. This keeps the test aligned with the
constructor's defaults and avoids duplicating field setup.

Auto-committed-on: dragonfly
Co-authored-by: Medulla <medulla@tinyhumans.ai>
Explain that when a storage backend is configured, approval store
operations run against the document port instead of approval.db, with
SQL guards expressed as preconditions and the desktop default unchanged.

Findings

  • medium · critique · Test expiry at sub-millisecond precision — This only exercises millisecond-aligned timestamps, so it cannot catch the remaining precision loss in the document store: `expires_ts` is stored and queried with `timestamp_millis (crates/openhuman\-core/src/security/approval/store\_documents\_tests\.rs:320)
  • medium · critique · Preserve sub-second precision in expiry timestamps — The query in `expire_stale` compares `now.timestamp_millis()` against this stored value, so expiry precision below one millisecond is discarded. An approval expiring at `...00.0009 (crates/openhuman\-core/src/security/approval/store\_documents\.rs:173)
  • medium · critique · Add a deterministic tie-breaker for recent decisions — When two decisions have the same `decided_at` value, this query supplies no secondary ordering. That can happen whenever callers use the same `decide_at` timestamp and is also poss (crates/openhuman\-core/src/security/approval/store\_documents\.rs:337)
  • medium · critique · Test approval behavior on both storage backends — Every test in this file constructs `Docs` over `MemoryStorage`, so the suite cannot detect regressions in the alternate approval storage path, including expiry publication and CAS (crates/openhuman\-core/src/security/approval/store\_documents\_tests\.rs:9)
  • high · security · Do not fall back to SQLite when document scope capture fails — `current()` returns an error when the storage scope cannot be resolved, including SaaS calls without an acting agent. Converting that error to `None` makes `WaiterGuard::drop` call (crates/openhuman\-core/src/security/approval/store\.rs:353)
  • medium · security · Preserve fractional precision when expiring document approvals — The document-backed path is now active, but `Docs::insert_pending` and `Docs::expire_stale` represent `expires_ts` with `timestamp_millis()`. An approval whose expiry falls within (crates/openhuman\-core/src/security/approval/store\.rs:280)
  • medium · security · Preserve sub-second precision when expiring approvals — The expiration index stores only millisecond timestamps, while `expires_at` retains nanosecond precision. `expire_stale` compares the similarly truncated current time against this (crates/openhuman\-core/src/security/approval/store\_documents\.rs:173)
  • medium · tests · Cover expiry publication through the public store on the document path — The only expiry exercised by the public store in this PR's e2e suite is a no-op (`expire_stale(&config).unwrap(), 0`). The `publish_expired` path reached from `store::expire_stale` (tests/storage\_approvals\_e2e\.rs:44)
  • medium · tests · Cover expiry with sub-second precision at the Docs level too — `sub_second_timestamps_order_and_expire_on_time` pins the fixed-precision format against whole-second vs sub-second ordering and expiry at a 900 ms due time, which is the main prio (crates/openhuman\-core/src/security/approval/store\_documents\.rs:234)

tests

  • Conclusion: Success
  • Scope reviewed: all assigned evidence
  • Lane summary: A prior review already went through store_documents.rs, store_flow_trust.rs and store.rs; the new commits re-add the same document-store implementation with most earlier complaints addressed in-revision (fixed-precision sortable timestamps with sub-second expiry tests, sanitize-before-cap asserted on the stored error, len-prefixed collision-free trust ids, deterministic ordering via decide_at, flow-trust key test) and add two genuinely new pieces: `capture_docs`/`decide_captured` so a parked approval's Drop denial still lands on the right store, and an end-to-end test that a configured backend replaces approval.db. The store-level regression tests fail if the document path loses its once-only decide/expire/record-execution guarantees, and the e2e test fails if the classic DB is used or bypassed. Remaining gaps are moderate: expiry publication on the document path is only covered indirectly, several new document behaviours (keep-decided purge, unreadable-decision, preauthorization collision) are asserted at the Docs unit level but never through the public store functions the e2e exercises, and the fallback-once-expired publication test that the public path needs still does not exist. With those noted as coverage findings rather than blockers, this is in good shape to merge. (31 earlier finding(s) still open) _Code retrieval was unavailable (model: ladder embeddings returned 400 Bad Request: {"error":{"message":"unknown ladder vectors; known ladders are flash (also chat-v1, flash-v1), instant (also no-think, instant-v1), reasoning (also deepseek), max-reasoning (also max-reasoning-v1), deepseek-flash (also reasoning-v1, agentic-v1), deep (also luna), scribe, uncensored, vectors-oai3 (also embeddings-oai3-v1), vision (also vision-v1, multimodal-v1), image (also images-v1, image-v1), vi), so this review saw the diff alone._ _Memory was unavailable (model: cortex: v1/recall: error sending request for url (http://cortexdb:3141/v1/recall\)\), so this review ran without it._

description

  • Conclusion: Success
  • Scope reviewed: all assigned evidence
  • Lane summary: The description now matches the change: approvals go to the configured storage backend's document store, scoped to the acting agent with a fail-closed SaaS rule, compare-and-swap guards, and the untouched SQLite desktop path, all covered by unit and e2e tests. The prior document-store findings (timestamp precision and sortability, expiry publication, sanitization, collision-free flow-trust keys, deterministic tests) are fixed in this revision; the one concern that remains is the capture-based decide path, which silently falls back to the SQLite store when no store was captured, and I could not verify the tinyagents Blocking port that block_on depends on from this diff. (31 earlier finding(s) still open) _Code retrieval was unavailable (model: ladder embeddings returned 400 Bad Request: {"error":{"message":"unknown ladder vectors; known ladders are flash (also chat-v1, flash-v1), instant (also no-think, instant-v1), reasoning (also deepseek), max-reasoning (also max-reasoning-v1), deepseek-flash (also reasoning-v1, agentic-v1), deep (also luna), scribe, uncensored, vectors-oai3 (also embeddings-oai3-v1), vision (also vision-v1, multimodal-v1), image (also images-v1, image-v1), vi), so this review saw the diff alone._ _Memory was unavailable (model: cortex: v1/recall: error sending request for url (http://cortexdb:3141/v1/recall\)\), so this review ran without it._

e2e

  • Conclusion: Neutral
  • Scope reviewed: all assigned evidence
  • Lane summary: The behavioural change — every approval store operation switching to the tinystoragedrivers document port when a backend is installed — is driven end to end by the new tests/storage_approvals_e2e.rs suite: it installs a backend, exercises insert/list/decide/audit/expiry/purge and flow-trust through the public store functions, and asserts the classic approval.db is not written and comes back when the backend is cleared. No earlier finding of mine was addressed by the new commits, but none of them concerned end-to-end coverage, so none carries into this lane. Waiting on end-to-end jobs: `Rust E2E (mock backend)`, `Build Playwright E2E Artifact`, `E2E (Playwright / web lane)`, `Desktop E2E (full suite, 3 OS)`. (31 earlier finding(s) still open)

📝 Walkthrough

Walkthrough

Approval persistence now supports a scoped document backend alongside SQLite. Storage APIs resolve scopes and run storage futures. Approval and flow-trust operations route through the configured backend, with tests covering document storage and SQLite fallback.

Changes

Approval Storage

Layer / File(s) Summary
Storage scope and blocking APIs
crates/openhuman-core/Cargo.toml, crates/openhuman-core/src/storage/*
The storage module adds scope resolution, scoped backend access, and a blocking bridge. Unit tests cover scope selection and bridge behavior.
Document-backed approval operations
crates/openhuman-core/src/security/approval/{mod.rs,store_documents.rs,store_documents_tests.rs}
The document store handles scoped approvals and flow-trust grants. Updates guard decisions, expiration, and execution recording. Tests cover these operations and their results.
Approval backend routing and cleanup
crates/openhuman-core/src/security/approval/{store.rs,store_flow_trust.rs,gate.rs,gate_intercept.rs}
Approval and flow-trust operations use the document store when configured and SQLite otherwise. Expiration publishes events. Cancellation cleanup uses the captured storage scope.
Backend integration and documentation
crates/openhuman-cli/Cargo.toml, crates/openhuman-core/src/security/approval/README.md, tests/storage_approvals_e2e.rs
The isolated integration test exercises the configured backend and SQLite fallback. The approval README documents backend selection and persistence details.

Merge Risk

Merge Risk: ⚪ Minimal · up to 9b893

Approvals now go through the scoped document backend when one is configured, and SQLite remains the fallback. No merge-blocking issue is evident in the supplied review context.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to 9b893

Agent-scoped storage strengthens approval isolation. However, switching back to the previous storage location can restore older flow permissions unless revocations are reconciled. The controls look sound for normal operation, but production cross-process failure behavior remains incompletely established.

Retained concerns

  • Medium · security · inferred: Flow-trust grants have separate document and SQLite authorities. If a previously granted flow/tool permission is revoked while document storage is active, subsequently removing that backend can reactivate the untouched SQLite grant and allow future calls without another approval prompt. This is a conditional operator-driven rollback risk, not a verified attacker-controlled bypass.

Security review details

Security Blast Radius

  • inferred — The rollback concern reaches flow/tool permissions previously stored in the restored SQLite workspace. A document-backed revocation can cease to govern future calls after an operator changes the active storage authority. The evidence does not establish arbitrary cross-tenant access or an attacker-controlled backend switch.

Security Findings and Attack Paths

  • inferred — The conditional authorization path is: retain an older SQLite flow/tool grant, revoke that permission in the active document store, remove the configured backend, then invoke the previously trusted flow/tool pair. The restored grant can short-circuit approval parking. This follows from the separate authorities; it was not demonstrated as an exploit.

Trust Boundaries and Controls

  • observed — Backend binding uses the acting agent's scope, with local scope allowed only outside SaaS when no agent is present. Scope-resolution or backend-binding errors propagate; SQLite fallback occurs when no backend is installed, not merely because document storage failed.
  • observed — External cancellation retains both the original scoped document handle and exact request ID. An unresolved captured scope remains an error instead of switching cleanup to SQLite or another agent's scope.

Resilience and Maintainability Implications

  • observed — Inspected test declarations cover one winner among concurrent deciders, separate approval and trust visibility between scopes, and cleanup with a captured handle. They provide useful counterevidence but use memory storage and were not executed during this review.

Hardening Proposals

  • proposed — Treat backend removal or replacement as an authorization-state migration: reconcile revoked flow grants, or require an explicit trust reset before the previous authority becomes active.
  • proposed — Validate scope isolation and terminal-state CAS against the shared production driver under interruption, ambiguous write outcomes, concurrent cleanup, and recovery. Keep the guarantee explicitly about durable decision and outcome recording unless external execution idempotency is separately established.

declare runs ensure_collection twice on every store operation.

run calls declare before each operation. On remote backends such as MongoDB, this adds index-creation round-trips to every approval check. Cache the declaration once per backend, for example with a OnceLock keyed by the backend.

Adds a document-backed approval store and wires the approval gate to
intercept requests before they proceed. This lets approvals be persisted
and checked consistently across the security layer.

…solved

The captured document store now carries the scope-resolution error instead of
silently falling back to SQLite, so a decision made after the task scope is
gone fails loudly rather than writing to the wrong store. Expiry timestamps are
also compared at nanosecond resolution, with a millisecond fallback beyond the
i64-nanosecond range, so approvals expiring within the same millisecond are
ordered correctly.

P2 Badge Preserve expiry events when a later CAS fails

When a configured document backend has several stale requests and a later CAS fails transiently, earlier update calls in this loop have already committed pending=false, but this ? exits without returning the accumulated expired rows. Callers therefore never invoke publish_expired for those approvals, and a retry cannot recover the events because the committed rows no longer match pending=true, leaving their approval surfaces stale until a full refresh. Publish each successful transition as it commits or return the partial successes alongside the error.

Assert the complete state of the expired approval

The expiry test checks only the request ID returned by expire_stale. It does not verify that the returned PendingApproval retains the tool name, summary, arguments, timestamps, source context, or tool-call ID that callers use when announcing the expiration. A regression that returns an incomplete or reconstructed record would pass this test; assert the returned record's full relevant state.

Cover ties in recent-decision ordering

This test deliberately gives every decision a distinct decided_at, so it only verifies lexicographic ordering when timestamps differ. list_recent_decisions sorts by decided_at alone; when two decisions are recorded in the same timestamp, the backend may return either order and the limit can select an unstable entry. Add a deterministic secondary key in the implementation and exercise equal-timestamp decisions in this test.

Exercise expiry publication through the public store

This calls Docs::expire_stale directly, so it only verifies the storage mutation. It does not verify the public document-backed decision path publishes each newly expired approval; a regression in that notification step would still pass. Add a public-store/captured-store case with an expired approval and assert the publication-facing behavior.

Drive execution and flow-trust operations through the backend API

These tests invoke the document implementation directly. They do not verify that the public backend-facing paths correctly route record_execution or flow-trust list/delete operations, so a wiring or scope-capture regression could ship while this suite remains green. Add backend-level coverage for those operations rather than testing only Docs methods.

Initialize the captured storage field

Adding this required field means every ParkingGuard struct literal must now provide docs. The diff adds the field and reads it in Drop, but does not update the existing construction path to call store::capture_docs(), so the crate will fail to compile with a missing-field error. Initialize the field wherever the guard is created, preserving the unresolved-scope error instead of silently falling back.

Add a deterministic tie-breaker for recent decisions

Recent decisions are ordered only by decided_at. Multiple decisions can legitimately share the same RFC3339 nanosecond timestamp, especially through decide_at, and the document backend is then free to return them in arbitrary order. Add a stable secondary sort, such as the request ID, so pagination and audit display remain deterministic.

Expire stale approvals before reading their decision

This reads the document directly and only returns a decision when decided_at is already present. An expired pending approval therefore returns None instead of being atomically denied and published as an expired approval, unless every caller separately performs expiry first. Make the document-backed get_decision path expire stale approvals before inspecting the request, matching the SQL-backed behavior.

Drive record_execution and flow-trust list/delete on the backend e2e

The new end-to-end test routes insert_pending, decide, expire_stale, list_recent_decisions and purge_session through a backend, but three public store surfaces with a document path are still only exercised at the unit level: record_execution (Docs::record_execution), list_flow_trust (used by the save-time manifest diff), and delete_flow_trust (revocation). A test would have to decide a request, call store::record_execution with an outcome and an error containing a secret, then read the stored audit state — and insert a second flow trust, list it via store::list_flow_trust, and revoke via store::delete_flow_trust — all with the backend installed. Without that, a regression in those document paths would pass CI. (Kept from the earlier review; the surrounding e2e file is new, so the gap is now anchored here rather than on the store module.)
