What changed

New docs/hosted-upload.md: a plain-language page on exactly what the desktop app sends to the hosted card service, what it never sends, how it's stored, how often it sends and what disconnect does. Linked from the README docs list and hosted/README.md.

Why

#140 done-when: "Plain-language setup/privacy docs show exactly what leaves the PC." Every claim matches the code merged in #289, #295, #297 and #304.
