docs: add scope labels, docs/README.md index, and docs/historical/ - #43

Each doc now has an "Applies to:" callout at the top so readers who land directly on a file know immediately whether it's relevant to them. Adds docs/README.md as a lightweight index table (audience + purpose) for readers browsing the folder on GitHub. Updates the README's docs/ file listing with [snipemgr] / [integrations] / [both] scope tags.
- Renames docs/phases/ → docs/historical/ and moves order-of-operations.md into it — all frozen post-v1.1.0 build records are now in one place - Adds docs/historical/README.md with a file index - Adds docs/README.md as a scope-labeled index of all docs - Adds "Applies to:" callout at the top of every doc - Updates README and CONTEXT.md references to reflect new paths
