fix(hasla): zawęź formularz zmiany hasła, usuń nieprawdziwy alert - #938

Formularz rozciągał się na całą szerokość okna — owinięty jak formularz logowania (large-6 medium-8). Alert o wyświetleniu formularza logowania po zmianie hasła był nieprawdziwy: widok zachowuje sesję (update_session_auth_hash) i przekierowuje na password_change_done.
