feat(menu): add Claude and Codex usage controls

Adds a compact menu bar view for checking account usage and managing Claude Code and Codex accounts without opening Settings. Usage reporting is intentionally limited to these two providers because their management APIs expose the quota data needed by the app; all other providers retain their existing behavior in Settings.

• Shows five-hour, weekly, and model-specific usage limits when available.
• Supports per-account enablement, email privacy, manual refreshes, and Codex reset credits.
• Caches quota reads and limits manual refresh frequency to reduce provider requests.
• Moves existing verification coverage into XCTest so configuration and quota behavior run together in CI.
