docs: refresh self-evolution verification record (#55)

Summary

• rewrite the self-evolution verification record as an explicit post-docs: make file examples consistent with the active workspace boundary #54 historical snapshot
• record the discovered 130-test count and real isolated benchmark results with provider, model, protocol, and evidence status
• make the docs checker reject obsolete commit/test claims and require the current verification commands and benchmark metadata
