Both of these come from how the CSV exporter works. You didn't do anything wrong.

Duplicate rows. Accept All Predictions doesn't replace the predictions. It adds a user instance next to each one and links it back through from_predicted (AddUserInstancesFromAllPredictions). The GUI hides the linked prediction, but the CSV export (CSVAdaptor.write → sleap-io _to_instances_df) writes one row for every user instance and one for every predicted instance (dataframe.py). In your screenshot, the row with empty scores is your corrected user instance, and the row with scores is the original prediction. Frames with a single row have only one of the two.
