docs(planning): reconcile active plans, specs, and GitHub status #117

Description

@joshsadam

What to build

Reconcile active planning documents, feature specifications, and GitHub status so contributors can tell what is merged, awaiting external review, ready to implement, or historical.

This includes correcting stale pull-request and issue states, reconciling the implemented Toolbar work with its unchecked task list, removing or correcting references to planning files that do not exist, and clearly marking superseded audits and pending notes as historical or archived.

Pull requests authored by the maintainer must be described as awaiting external review and merge, not as work the maintainer can complete independently.

Acceptance criteria

  • Active planning status matches current GitHub issue and pull-request state.
  • The Toolbar specification accurately distinguishes implemented work from outstanding manual validation.
  • Active planning documents do not depend on missing roadmap, requirements, or state files.
  • Historical audits and superseded notes are clearly separated from current execution guidance.
  • Authored pull requests are consistently described as awaiting external review and merge.
