I polished and opensource'd my self-service tool where patients can create and cancel their appointments online:

While installing and running the program is quite easy, the complete setup needs some basic understanding of how the internet works. But probably someone will like it anyway. My patients certainly do ;-) (and my assistants have far less phone calls).
