Read questionnaire from Google Sheet

I think it'll make any automation easier if we can read the questionnaire straight from the Google Sheet in which it's maintained.

That was already the case last time I used it. You just need to pass the CSV export URL instead of a path, and it reads it from there. The only problem is that, if I recall correctly, the columns changed.
