fix: split the tracker-leak scanner's own internal-hostname literal
The internal-hostname rule added in this release carried the literal
domain string it exists to catch, so the pre-push hook correctly flagged
its own definition line as a new tracker leak the moment it was pushed.
Split the hostname into two concatenated parts (same technique already
used for the Jira-key/Atlassian-host fixtures in
packages/opencode/test/skill/tracker-leak-check.test.ts) so the rule's
source no longer contains the contiguous literal it matches against.
